Facilities Request Form
If you would like to use/rent our facilities for your event, please complete the following form. Once this request is received, you will be contacted regarding the approval or denial of your event. If approved, you will receive an estimated cost of the event for your rental. If you have any questions about rentals, please contact our Building Administrator, Ann Cottone (ann@chbcky.org).

All set-up needs must be provided no later than 14 days prior to the event. Set-up requirements not received by then cannot be guaranteed.

*Audio /Visual is only available for events in Fellowship Hall and the Sanctuary. *The Chapel is audio only.

*CHBC Members - only administrative and after hours custodial fees apply - room rental costs will not apply.
**Missions Partners of CHBC - pay 50% of hourly rental fee for any after hour custodial work. Office Hours: Monday - Friday, 8:30 AM - 4:00 PM

Entry prior to this time cannot be guaranteed. Earlier arrival must be communicated at least 2 weeks prior to your event. If you access the space prior to the time written here, you will be billed for the additional time.

Event Space Requirements: If you know which location(s) you will need for your event, please indicate them below. If you are unsure, please mark "Undetermined." You will then need to schedule a time to view the event spaces available to best determine the location you need.

Chapel - $150/hour - After hours custodial fee: $35/hour
Fellowship Hall - $150/hour - After hours custodial fee: $35/hour
Sanctuary - $195/hour - After hours Custodial fee: $35/hour
Concerts in the Sanctuary - $500 - After hours Custodial fee: $35/hour
Heritage Hall - $75/hour - After hours Custodial fee: $35/hour
Birchwood Lobby - $75/hour - After hours Custodial fee: $35/hour
Gym - $50/hour - After hours Custodial fee: $35/hour *The Gym is not handicap accessible
Classrooms - $50/hour - After hours Custodial fee: $0/hour
Parking Lot (*select this option if your event will be held in the parking lot) - $50/hour
Undetermined - Meeting needed to determine space.
*1-hour minimum rental each with hourly room rental cost capped at 4 times the rate for a full day of use. Hourly rate includes room access before and after the event. Multiple room rentals for the same event receive a $25 discount on the additional rooms. Audio/visual equipment is available for use, by onsite staff support is available for an additional $200 fee during the event.
Wedding fees are covered by a separate detailed policy.
